How Effective is Tadalafil (Cialis)?
The safety and efficacy of Tadalafil in the treatment of erectile dysfunction has been evaluated by questionnaires in 22 clinical trials involving more than 4,000 men. Included in these questions were whether or not the participant was able to achieve an erection long enough to allow for vaginal penetration as well as if they could maintain an erection long enough for full sexual intercourse.
In all trials, Tadalafil proved to be significantly better than the placebo in improving men’s ability to achieve and maintain erections.
How Is Tadalafil (Cialis) Administered?
The recommended initial dose of Tadalafil for most patients is 10 mg taken orally approximately 1 hour before sexual activity. The dose may be increased to 20 mg or decreased to 5 mg depending on a variety of factors. Tadalafil should be taken once a day and can be taken on an empty or full stomach. The effects of Tadalafil can last up to 36 hours.
What Are the Side Effects of Tadalafil (Cialis)?
The most common side effects reported include: headaches, indigestion, back pain, muscle aches, facial flushing, and nasal congestion. All side effects were minor in frequency and severity.
Back pain and muscle aches occurred in less than 7% of patients, and usually occurred at 12 to 24 hours after taking Tadalafil. The back pain and muscle aches associated with Tadalafil were described as mild to moderate muscle discomfort in the lower back, buttocks and thighs, often aggravated by lying down. The back and muscle aches tended to go away within 48 hours without any treatment. Most pains went away after taking some Tylenol or Motrin. Less than one half of one percent discontinued their use of the drug because of this side effect.
Reports of abnormal vision were rare and occurred in less than 0.1% of patients using Tadalafil.
Though rare, there have been reports of priapism (prolonged and painful erections lasting more than 6 hours) with the use of PDE5 inhibitors such as Sildenafil, Vardenafil, and Tadalafil. Patients with blood cell diseases such as sickle cell anemia, leukemia, and multiple myeloma have above average risks of developing priapism. Priapism, if left untreated, can cause injury to the penis and lead to permanent impotence. Contact a doctor immediately if you experience an erection longer than 4 hours.
Who Should Not Use Tadalafil (Cialis)?
Tadalafil can cause hypotension (abnormally low blood pressure that can lead to fainting and even shock) when given to patients who are taking nitrates. Because of this, it is recommended that all patients taking nitrates on a daily basis should not take Tadalafil.
Tadalafil should not be used with alpha-blockers, medicines used to treat high blood pressure or benign prostate hypertrophy (BPH), because this combination of Tadalafil and an alpha-blocker may lower the blood pressure greatly and lead to dizziness and/or fainting. Examples of alpha-blockers include: Tamsulosin (Flomax), Terazosin (Hytrin), Doxazosin (Cardura), Alfuzosin (Uroxatrol), and Prazosin (Minipress). The only alpha-blocker that can be used safely with Tadalafil is Tamsulosin (Flomax). When Tadalafil (20 mg) was given to healthy men taking 0.4 mg of Flomax daily, there was no significant decrease in blood pressure indicating that there is no increased risk taking these two drugs simultaneously. The only alpha blocker that has not been tested with Tadalafil is Alfuzosin (Uroxatrol), and thus, there is no data to indicate whether there is a negative reaction between the two. Tadalafil should not be used with alpha-blockers (except Flomax), medicines used to treat high blood pressure and benign prostate hypertrophy (BPH), due to the combination of Tadalafil and an alpha-blocker lowering the blood pressure greatly and leading to dizziness and/or fainting. • Angina (chest pain due to coronary artery disease)
• Low blood pressure (a resting systolic blood pressure less than 90mm Hg)
• Uncontrolled high blood pressure (greater than 170/110 mm Hg)
• Recent stroke or heart attack (within the last 6 months)
• Uncontrolled, potentially life-threatening, abnormal heart rhythms
• Severe liver disease
• Severe heart failure or disease of the heart’s valves, for example, aortic stenosis
• Retinitis pigmentosa
Men suffering from these conditions should not use Tadalafil without having these conditions evaluated and stabilized first. A doctor should also test the heart using a cardiac treadmill test to ensure that the heart is strong enough to handle the extra workload required for sexual activity.
What Precautions Should Be Taken When Using Tadalafil?
For most healthy men, trace amounts of the drug will remain in the body for more than 2 days after a single dose of Tadalafil. The metabolic breakdown (clearing of the drug from the body) of Tadalafil can be slowed by liver disease, kidney disease, and the concurrent use of certain medications (such as erythromycin, ketoconazole, and protease inhibitors). This causes the drug to stay in the body longer and could cause unwanted side effects. Therefore, the dose and frequency of Tadalafil should be lowered in the case of the following conditions:
• Medications such as erythromycin, Ketoconazole (Nizoral), Itraconazole (Sporanox), Ritonavir (Norvir), and Indinavir (Crixivan) can slow the breakdown of Tadalafil. Therefore men taking these medications should not take more than 10 mg of Tadalafil in a 24 hour period
• Men with moderately severe kidney impairment should initially take a dosage of 5mg every 24 hours, and not to exceed the maximum dose of 10 mg taken every 48 hours. In men with severe kidney disease and on dialysis, the maximum dose should not exceed 5 mg
• Men with severe liver disease should not take Tadalafil. Men with mild to moderate liver disease should not exceed 10 mg every 24 hours
As with all medications there will be side effects that are unwanted. Some of these side effects can be guessed based on your pre-disposition to a variety of factors. But, there will always be unknown side effects that are not anticipated. It should be said that though side effects may occur, it is not necessarily a reason to stop taking a certain drug or give up on a treatment. These side effects are often temporary and can go away when the dosage is changed. Diabetes Impotence

Estimates show that about 35-75% of men with diabetes will experience at least some degree of erectile dysfunction, also known commonly as impotence, during their lifetime. This is much higher than the percentage of men who are not afflicted with diabetes.
Men with diabetes tend to develop erectile dysfunction up to 10 to 15 years earlier than men without diabetes. As men with diabetes age, erectile dysfunction becomes even more probable. Those over the age of 50 have an increased likelihood of having difficulties with an erection and it has been shown to occur in approximately 50-60% of men with diabetes. Those over the age of 70, show a 95% likelihood of having some difficulty with erectile function.

Why Do Men With Diabetes Have Impotence?

The causes of erectile dysfunction in men with diabetes are complex and normally concern problems in the nerves, blood vessels and muscle function.
In order to achieve an erection, healthy blood vessels, nerves, male hormones, and a desire to be sexually stimulated are needed. Diabetes can damage the blood vessels and nerves that control erection. Unfortunately, even if you have normal amounts of male hormones as well as the desire for sex, a firm erection might still not be possible because of one of the other functions is not working properly. Diabetes affects so many different systems in the body that even one is diagnosed as having mild diabetes, the effects on one’s sex life can still be great.

Which Treatments Are Available for Men With Diabetes and Impotence?

In some cases the side effects can be minimal, as with the penis vacuum mechanisms, but the psychological factors of feeling inadequate might be too much for a patient to take. And, on the other side, though pills like Viagra are normally the easiest treatment psychologically, the physical side effects can be great if you have a pre-existing condition like diabetes.

If you have ever watched the Super Bowl or the World Series, chances are you have already seen an advertisement of Viagra, Levitra, or Cialis. You probably already know the basic information about these three medications, but there is actually a lot more to know in order for you to select which is the one that will work best for you.

All three medications work in the same way. The drugs are all phosphodiesterase type 5 or PDE-5 inhibitors. PDE-5 is an enzyme that breaks down cGMP, which is the chemical responsible for relaxing the muscles along the penis and increasing blood flow in the penis – in effect, creating an erection. The three erectile dysfunction medications all work towards inhibiting PDE-5, which will then keep cGMP levels high enough to supply adequate blood to the penis to create an erection. This also works in women by relaxing the clitoral smooth muscles, also allowing blood to flow into the clitoris.

The question still remains – which of the three medications is the best? As of this writing, no studies have yet been conducted to compare the effectiveness, side effects, or food interactions of all three PDE-5Is.

Some people claim that Levitra shows results faster than Viagra, and that it can be absorbed better after a meal. All three drugs produce the same side effects except for one or two differences. All three drugs are also not to be mixed with any kind of nitrate. It is also not advisable to mix PDE-5Is with doxazosin, but in Cialis, it is strictly prohibited.

What causes erectile dysfunction (ED)?

The ability to achieve and sustain erections requires several things, including:
1. A healthy nervous system that is able to conduct nerve impulses in the brain, spinal column, and penis
2. Healthy arteries in and near the Corpora cavernosa,
3. Healthy smooth muscles and fibrous tissues within the Corpora cavernosa
4. Adequate levels of nitric oxide in the penis.
Should one or more of these characteristics not be met, erectile dysfunction can occur.

The causes of erectile dysfunction are as follows:

• Aging

There are two reasons why older men are more likely to experience erectile dysfunction than younger men. Firstly, older men are more prone to diseases such as heart attacks, angina, strokes, diabetes mellitus, and high blood pressure that are often times associated with erectile dysfunction. Secondly, erectile dysfunction can be caused simply by the natural process of aging, primarily by decreasing the compliance of tissues in the Corpora cavernosa. Though it has been suggested, it has not yet been proven that aging causes a decrease in the production of nitric oxide in the nerves that innervate the smooth muscles within the penis.

• Diabetes mellitus

On average, erectile dysfunction tends to develop 10 to15 years earlier in diabetic men than among men without signs of diabetes. In a random population study, which took men who have suffered from type I diabetes for more than 10 years, erectile dysfunction was reported by 55% of the men between the ages of 50 to 60 years old. This increased risk of erectile dysfunction among men with diabetes mellitus could be due to the earlier onset and greater severity of atherosclerosis that has been shown to limit the blood flow by narrowing the arteries and thereby limiting the blood flow to the penis. Without sufficient blood flow to the penis, an erection is simply not possible to achieve.
Diabetes mellitus has also been proven to cause erectile dysfunction by means of damaging both sensory and autonomic nerves; this condition is known as diabetic neuropathy. Smoking cigarettes, obesity, poor control of blood glucose levels, and having diabetes mellitus for a prolonged time further increases the risk of erectile dysfunction in men. Additionally, atherosclerosis and/or neuropathy causes ED in men, making many diabetic men develop a myopathy (muscular disease) as the cause of ED. This myopathy can cause a decrease of the compliance of muscles in the Corpora cavernosa which in turn causes the inability to initiate or maintain an erection. • Hypertension (high blood pressure)

Through studies, it has been shown that patients with essential hypertension or arteriosclerosis have an increased risk of developing erectile dysfunction. Essential hypertension is the most common form of hypertension getting its name from the fact that it is not caused by another disease (unlike other types of hypertension which can be caused by kidney disease). It is not clearly known how essential hypertension is connected with erectile dysfunction. However, patients with essential hypertension have been found to have low production of nitric oxide by the arteries of the body, including the necessary arteries in the penis. For this reason, scientists now contribute erectile dysfunction to the decreased levels of nitric oxide which is caused by essential hypertension.

• Cardiovascular diseases

In the US, the most common cause of cardiovascular disease is atherosclerosis. Atherosclerosis is defined as the narrowing and hardening of arteries causing the reduction of blood flow and increasing blood pressure. It typically affects many arteries throughout the body, and has been proven to be aggravated by: hypertension, high blood cholesterol, smoking, and diabetes mellitus. Heart attacks are the result when coronary arteries (arteries that supply blood to the heart) are narrowed due to atherosclerosis. Should the same blockage of arteries caused by atherosclerosis affect the cerebral arteries, the result is a stroke in the brain.
This same logic holds true for the penis. When arteries are narrowed due to atherosclerosis, it causes much needed blood flow to not reach the penis and/or pelvic organs, problems can occur. Should there be inadequate blood flow to the penis, the lack of erection is the result. Research shows a very strong correlation between erectile dysfunction and the severity of atherosclerosis in the coronary artery. Thus, men who have a more severe atherosclerosis in the coronary artery tend to have a greater chance of having some type of erectile dysfunction. And, in turn, those with less severe cases tend to have a lower chance erectile dysfunction. Several panels of doctors have suggested men who have just been diagnosed with onset erectile dysfunction be immediately checked for coronary artery diseases (advanced coronary artery atherosclerosis that has not yet caused angina or a heart attack). • Cigarette smoking:

• Nerve or spinal cord damage:

Damage to the spinal cord and nerves in the pelvis can cause erectile dysfunction. Nerve damage can be caused by disease, trauma, or surgical procedures. Examples of damage include: injury to the spinal cord from an accident (automobile, falling on head), injury to the pelvic nerves from prostate surgery, multiple sclerosis (a neurological disease causing widespread nerve damage), and long-term diabetes mellitus.

• Substance abuse:

Marijuana, heroin, cocaine, and alcohol abuse have been proven to contribute to erectile dysfunction. Alcoholism, in addition to causing nerve damage, can lead to the atrophy of the testicles and lower testosterone levels. For more information, please visit the Drug Abuse Center.

• Low testosterone levels:

Testosterone (the primary sex hormone in men) is not only necessary for sex drive (libido) but also is necessary to maintain nitric oxide levels in the penis. Therefore, men with hypogonadism (diminished function of the testes resulting in low testosterone production) can suffer from low sex drives and erectile dysfunction.

Male Impotence

The inability to achieve or maintain a sufficient penile erection throughout sexual intercourse or other sexual activities is the most commonly accepted definition of impotence. Ejaculation (cumming) may or may not be present in impotent males during sexual activities. Impotence is also known as erectile dysfunction (ED) in the scientific community. Impotence can be categorized under two different levels: primary and secondary.

Primary Impotence:

a male has never had successful intercourse with a partner but, under other circumstances, has achieved a full erection.

Secondary Impotence:

a male has problems with successful full erections, but has had a history of having successful erections in the past.

How Common Is Impotence?

In their lifetime, most men will occasionally experience impotence, usually a result from stress, tiredness, anxiety or excessive consumption of drugs, alcohol, or a mixture of both. Many times, the psychological fear of not having an erection (known as “fear of failure”) may cause the symptoms of impotence to worsen due to the performance anxiety that one causes themselves. Research has established how prevalent ED is among men. The following are the age ranges and percentages showing how likely it is that one might have troubles with impotence:
- 13.1% of 40-49 year olds
- 33.5% of 50-59 year olds
- 51.5% in 60-69 year olds
- 69.2% of 70-79 year olds
Unfortunately, statistics show that many men never receive any treatment against impotence, or wait a prolonged amount of time before seeking treatment due to this psychological “fear of failure.” It has been shown, however, that erections can change over the course of one’s life, sometimes being strong, while other times being quite weak and almost non-existent. A male teen often has little problem, or control, with having an erection. Often times this leads to embarrassing situations as a teen will have an erection at a time that no sexual activity is happening. For most men, this embarrassing situation stops once the teen reaches their late teens or early twenties. As a male gets older, erections might not come as quickly as they had in the past. Every man, at one point in there life, has had an erection that is less strong than they would have preferred, however, at some point there it can be defined as the medical problem known as impotence.

Though this subject used to be taboo, it is now a very widely discussed topic. Men have always suffered from impotence, but in the past it was always viewed as a sign of weakness or impotence in all areas. Thankfully, through our evolving as a society this is now not the case. Step 1

Experiment with exercises. Sometimes the key to lasting longer and achieving an erection lies in simply strengthening the muscles in the penis.
Try the following exercise: the next time you urinate, squeeze the muscle in your penis that stops the flow of urine (without using your hand). This is the same muscle needed for stronger bigger erections. Work it in the same way you would work any other muscle by means of a series of progressive exercises. Try progressively contracting the muscle for longer and longer periods and repeat throughout the day.
